RESUMO

Objective:To observe the effect of total knee arthroplasty in the treatment of knee osteoarthritis.Methods:From April 2017 to April 2019, 80 patients with knee osteoarthritis admitted to Zhejiang Rongjun Hospital were selected in the research.The patients were divided into control group(40 cases) and observation group(40 cases) according to the random digital table method.The control group was operated by arthroscopy, and the observation group was operated by total knee replacement.The bleeding volume, incision healing time, hospitalization time, incidence of postoperative complication, knee joint activity, VAS score and HSS score were compared between the two groups.Results:The amount of bleeding in the observation group was (137.26±30.85)mL, which was less than that in the control group[(300.31±58.40)mL]( t=15.613, P<0.001). The healing time of incision and hospitalization time in the observation group were (4.20±1.37)d and (7.58±1.20)d, respectively, which were shorter than those in the control group[(6.89±2.43)d and (10.50±3.20)d], the differences was statistically significant( t=6.099, 5.404, all P<0.001). The incidence of postoperative complications in the observation group was 7.50%(3/40), which was lower than 25.00%(10/40) in the control group(χ 2=4.501, P<0.05). The knee joint mobility of the observation group was (106.71±15.38)° after operation, which was higher than that of the control group[(95.51±11.62)°]( t=3.675, P<0.001). The VAS score and HSS score of the observation group were (2.50±0.68)points and (89.27±5.65)points, which were better than those of the control group[(4.53±0.71)points and (69.39±5.41)points], the differences were statistically significant( t=13.059, 16.073, all P<0.001). Conclusion:Total knee arthroplasty is effective in the treatment of knee osteoarthritis, which is helpful for the recovery of knee function.

Objective@#To investigate the neurocognitive characteristics and related factors in the elderly with subjective cognitive decline.@*Methods@#Among the 1 850 elderly volunteers aged over 50, 377 cognitive normal elderly (NC group), 234 subjective cognitive decline (SCD Group) and 291 patients with mild cognitive impairment (aMCI Group) were screened with the brief elderly cognitive screening questionnaire and the elderly rapid cognitive screening scale.They were all received clinical interview and examination and core neurocognitive test.@*Results@#(1) There were statistically significant differences in the three groups on the age, education, occupation, HAMD, low density lipoprotein and blood pressure (P<0.01). (2) The score of the picture-symbol association in SCD group(8.94±4.05)was lower than that in NC group(9.83±4.18)and higher than that in aMCI group (7.12±4.17)(all P<0.05), while the scores of the other neuropsychological tests were higher than those in aMCI group.There were no statistically significant difference between SCD group and NC group on the other neuropsychological tests(P>0.05). (3)The SCD was mainly influenced by age(β=0.063, OR=1.065, 95%CI=1.033-1.099), depression(β=0.182, OR=1.199, 95%CI=1.084-1.327)and hypertension(β=0.473, OR=1.604, 95%CI=1.185-2.171)(all P<0.01). And the aMCI was mainly influenced by age(β=0.078, OR=1.081, 95%CI=1.048-1.115), education(β=-0.174, OR=0.840, 95%CI=0.778-0.907), occupation(β=-0.406, OR=0.666, 95%CI=0.535-0.830)and low density lipoprotein(β=-0.451, OR=0.637, 95%CI=0.497-0.816)(all P<0.01 ).@*Conclusion@#Objective neurocognitive function of the elderly with subjective cognitive decline is basically normal.Age, depression and hypertension are risk factors of subjective cognitive decline.

Conclu-sion Objective neurocognitive function of the elderly with subjective cognitive decline is basically normal. Age,depression and hypertension are risk factors of subjective cognitive decline.

Septin 9 (SEPT9) gene, a member of the conserved framework protein genes family, has guanosine triphosphataseg (GTPase) activity and play an important role in a wide range of biological processes such as cell division, cell polarization and membrane remodeling.Accumulated evidence have confirmed that SEPT9is closely related to the generation and development of human diseases.And SEPT9has become the new aroused general interest in tumor related research at this stage.This article mainly reviews the structural features of SEPT9gene, action mechanism of SEPT9protein and the role of SEPT9gene in a variety of common oncogenesis, and explores the potential value of SEPT9as a marker in early tumor screening.

Objective@#To discuss the application of three dimentional(3D)visualization technologies in treatment plan of hepatic malignant tumor.@*Methods@#The clinical data of 300 patients with liver malignant tumor who received treatment from January 2016 to January 2017 in the Third Department of Hepatic Surgery of Eastern Hepatobiliary Surgery Hospital was retrospectively analyzed in this study, including 221 male and 79 female patients aged from 7 to 76 years with median age of 54 years. The median height was 168 cm (115-183 cm), the median weight was 65 kg (20-105 kg) and the median tumor volume was 142 ml (23-2 493 ml). Three-dimensional visualization technology was used in all patients to reconstruct liver three-dimensional graphics. Also, two and three-dimensional methods were taken respectively to evaluate patients and develop treatment strategy. The change of treatment strategy caused by 3D evaluation, actual surgical plan, operation time, time of hepatic vascular occlusion, intraoperative blood loss, volumes of blood transfusion and postoperative complications was observed.@*Results@#After three-dimensional visualization technology was applied, 75(25%) of 300 patients′ treatment strategies had been changed. The range of hepatectomy was extended in 25 patients. And 7 of them were due to hepatic venous variation, which resulted in increasing drainage area. In other 4 patients, liver resections were extended due to lack of perfusion of the liver parenchyma after the removal of portal vein. And hepatectomy was expanded in 14 patients in order to increase the surgical margin. The range of hepatectomy was reduced in 8 patients, 4 of which were due to hepatic venous variation, such as hepatic vein of segment 4 or lower right posterior hepatic vein. The remaining 4 cases were because of insufficient residual liver volume.The surgical resection was performed in 278 cases, 257 of which received operation directly. Left hepatectomy was performed in 24 patients and right hepatectomy was performed in 33 patients. Left trisectionectomy was carried out in 12 patients and right trisectionectomy was carried out in 11 patients. Caudate lobectomy was applied in 10 patients. There were 18 cases of left lateral sectionectomy, 7 cases of right anterior sectionectomy, 25 cases of right posterior sectionectomy and 18 cases of mesohepatectomy. Single or multi segment resection was performed in 99 patients. The treatment strategy of thirty-six patients was converted to staged hepatectomy (ALPPS 11 cases and portal vein embolization 25 cases). The median operation time was 130 minutes (90-360 minutes) and the median inflow blood occlusion time was 20 minutes (0-75 minutes). Median blood loss volume was 200 ml (20-1 600 ml). Thirty-seven of 278 patients received transfusions, and the average red blood transfusion volume was (4.4±1.7)units (0-8 units). Median hepatic resection volume was 530 ml(30-2 600 ml). There were 117 cases of pleural effusion after operation, including 3 patients needing invasive therapy. Ascites occurred in 23 patients, 6 of whom needed invasive therapy. Biliary leakage was observed in 30 patients. Eight patients occurred hepatic cutting surface hemorrhage, 6 of whom received blood transfusion, and 4 of whom underwent laparotomy to stop bleeding. Three patients had pulmonary infection after surgery and 3 patients appeared biliary obstruction. Deep vein thrombosis took place in 2 patients and portal vein thrombosis was observed in 4 patients. No postoperative liver failure and death ever happened in our study group.@*Conclusion@#Three-dimensional visualization technique can optimize the treatment strategy of patients with liver malignant tumor, improve surgical safety.

AIM:To explore the influence of exogenous hydrogen sulfide ( H2 S) on coagulation and fibrinoly-sis in ferric chloride ( FeCl3 )-induced mouse carotid artery thrombosis .METHODS: The mice were divided into sham control group, model group, different concentrations (12.5, 25 and 50μmol/kg) of sodium hydrosulfide (NaHS, H2S do-nor) groups and 30 mg/kg clopidogrel ( positive control ) group.Intraperitoneal injection of NaHS at different concentra-tions and oral administration of clopidogrel bisulfate were performed for 3 d prior to FeCl 3-induced carotid artery thrombo-sis.The frozen sections of the carotid artery were collected to perform HE staining , and the thrombus pattern and the chan-ges of vascular pathology were observed .The thrombus was weighed to calculate thrombosis inhibitory rate .Prothrombin time ( PT) , activated partial thromboplastin time ( APTT) , fibrinogen ( FIB) and fibrinogen degradation product ( FDP) in the mice were also measured by a coagulometer .The plasma levels of thromboxane B 2 ( TXB2 ) , 6-keto-prostaglandin F 1α(6-keto-PGF1α) and plasminogen activator inhibitor (PAI) were detected by ELISA.RESULTS: Compared with model group, NaHS dose-dependently inhibited the formation of carotid artery thrombus .NaHS treatment reduced the contents of TXB2 and PAI, and recovered 6-keto-PGF1αcontent in thrombosis model group .In NaHS treatment groups , 6-keto-PGF1α/TXB2 and thrombus weight was negatively correlated .NaHS treatment prolonged PT and APTT , reduced the content of FIB, but increased the level of FDP in thrombosis model group .CONCLUSION:Hydrogen sulfide prevents FeCl 3-induced carotid artery thrombosis by inhibiting coagulation and activating fibrinolysis .

Based on the theory of the rise and fall of qi-blood in meridians, Professor GAO Yu-chun harmonizes the qi-blood and yin-yang for diagnosis and treatment of hypertersion from liver, spleen and kidney. With reference of the academic idea of“Preventive Acupuncture”, he investigated the qi and blood, treated diseases in the upper part by managing the lower part, pay attention to the syndrome differentiation of excess and deficiency, the different of acupoint property, and the treatment of acupuncture or moxibustion. Thus, through balancing yin and yang, the qi and blood are regulated to reach the therapeutic aim of“yin and yang in equilibrium”.

Objective To investigate the clinical outcome of cervical expensive open-door laminoplasty combined with vitamin B12 treatment for patients of cervical spondylotic myelopathy with subacute combined degeneration of the spinal cord.Methods Data of 10 patients with cervical spondylotic myelopathy with subacute combined degeneration of the spinal cord in February 2013 to August 2013 were retrospectively analyzed.There were 7 males and 3 females with an average age of 46 years (range,32-73 years).Eight patients' history was in six months,and others' was beyond six months.All patients had undergone cervical expansive open-door laminoplasty with mini-plate fixation combined with intramuscular injection or pre os vitamin B 12.Patient imaging data were used to assess the stability of the cervical spine,and spinal cord compression in the fixed position before and after their surgery.Japanese Orthopaedic Association (JOA) score was used to evaluate function,and the rate of neurological improvement was calculated.The American Spinal Injury Association was used to evaluate the neurological function inpatients before and after surgery.The preoperative and postoperative numerical changes of SEP and vitamin B 12 were compared using correlation analysis.Each dimension of preoperative and postoperative scores change was compared by Life quality evaluation F-36 scale.Results All patients were followed up for an average period of 15 months (range,6-26 months).There were no complications related to the internal fixation,such as fixation loosened or broken.Cerebrospinal fluid surrounding the spinal cord within the C3-7 range of the signal on MRI T2WI of the continuous had been recovered.JOA score increased from preoperative 9.6±2.04 to postoperative 14.7±2.04,and postoperative neurological improvement rate was 20%-80%.The excellent and good rate was 90% (9/10) including 4 excellent and 5 good.Vitamin B12 level before operation:78.30±42.82 pg/ml,postoperative:176.50±85.77 pg/ml.Electromyography and SEP had been all recovered.There was correlation between preoperative and postoperative vitamin B 12 levels with spinal cord function score.According to ASIA score,there were 4 cases in C level,6 in D level preoperatively,while 2 in C level,5 in D level,3 in E level postoperatively.Conclusion Cervical spondylotic myelopathy with subacute combined degeneration of the spinal cord could be treated by surgery.Vitamin B 12 by per os or intramuscular injection with removal of the cervical spinal cord compression which creates a space for recovery of spinal cord could improve the surgical curative effect.

Objective To design a multi-function hemostatic bandage for the hemodialysis patient so as to solve the problems during compression hemostasis after hemodialysis.Methods The bandage was composed of a belt and a monitoring box.The sensors for errhysis and pressure on the belt monitored the state of compression hemostasis at the puncture point after hemodialysis,and the devices of timing and alarming in the box reminded the staff the duration and pressure of compression.Results The bandage avoided massive errhysis by reminding the patient and staffs in time,and also solved the problems of overlong compression time and improper compression pressure.Conclusion The bandage gains advantages in low cost,easy operation,high sensitivity,and thus is worthy promoting clinically for hemostasis at puncture point after hemodialysis.

Objective:To observe the effect of Governor Vessel-unblocking and mind-calming acupuncture on insomnia and further develop the academic idea on Gao’s lineage acupuncture. Methods:A total of 68 insomnia cases were randomly allocated into an acupuncture group (n=34) and a medication group (n=34) according to their sequences of consultation. Patients in the acupuncture group received acupuncture treatment once a day, 5 d a week, for 4 weeks. Patients in the medication group took 1 mg of Estazolam before sleep for each dose, one dose a day, for 4 weeks. Before and after treatment, the sleep quality was evaluated using the Pittsburgh sleep quality index (PSQI), and the efficacy was assessed using the PSQI reductive rate. Results:After treatment, the global PSQI score and scores of each component in the acupuncture group were significantly decreased (allP<0.01). The global PSQI score in the medication group was significantly decreased (P<0.01). Except for sleep disturbance, scores of the rest of components were significantly decreased (P<0.01 orP<0.05). The between-group difference was statistically significant in global PSQI score (P<0.01). Except for sleep latency and duration, scores of the sleep quality, efficiency, disturbance and daytime dysfunction in the acupuncture group were significantly lower than those in the medication group (allP<0.05). In addition, the recovery and marked effect rate in the acupuncture group was significantly higher than that in the medication group (P<0.05). Conclusion:The Governor Vessel-unblocking and mind-calming acupuncture is safe, reliable for insomnia and has better efficacy than Estazolam.

Objective:To observe the clinical effect of Governor Vessel-regulating and brain-unblocking acupuncture on post-stroke depression. Methods:A total of 70 eligible cases were randomly allocated into an observation group and a control group according to the random number table. Patients in the observation group were treated with Governor Vessel-regulating and brain-unblocking acupuncture, whereas patients in the control group were treated with oral Escitalopram Oxalate tablet. The treatment lasted for 8 weeks. The Hamilton depression scale (HAMD), Montgomery Asberg depression rating scale (MADRS), and Barthel Index (BI) were used to evaluate the patients’ condition before, 4 weeks and 8 weeks after treatment respectively. In addition, adverse reactions were also recorded. Results:After 4 weeks of treatment, the HAMD and MADRS scores were significantly decreased in both groups (P0.05); the MADRS score in the observation group was significantly lower than that in the control group (P0.05); however, the adverse reactions in the observation group were significantly less than those in the control group. Conclusion:The Governor Vessel-regulating and brain-unblocking acupuncture is safe and effective for post-stroke depression and can obtain similar efficacy as Escitalopram Oxalate tablet. In addition, it can fast alleviate core symptoms of depression and improve the activities of daily living (ADL) and has fewer adverse reactions.
